Child new patient registration

Use this service to register a child with the surgery.

You can use this service if you:

  • want to register your child at the surgery
  • live within our catchment area
  • are registering a child under 16 years old

To register an adult aged 16 or over with the practice, fill out a adult new patient registration form.

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
  • your NHS number
  • information about your health
  • other personal details and demographics

Once you have submitted your registration form, the surgery will reply via email. You should receive an email within 3 working days. If you have not had a reply please check your junk mail.
