Adult new patient registration

Use this service to register an adult with the surgery.

You can use this service if you:

• want to register at the surgery
• live within our catchment area
• are 16 years old or over

To register a child under 16 years of age with the practice, fill out a child new patient registration form.

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• your NHS number
• details of previous GP surgery
• basic health and medical information

Once registered, make an appointment to attend the surgery for a new patient health check at your earliest convenience.
